Together Senior Health is interested in hosting a pilot online creative arts engagement activity for people living with dementia and their caregivers. We are seeking a Creative Arts professional who has creative engagement experience with people living with dementia and their caregivers. The program will be offered to a small group (6-12) of current Together Senior Health Moving Together program participants and take place on the TSH proprietary audio/video platform. For this first program, we would like participants to be able to use materials they would likely already have in their homes. Dementia Care Coach- Institute on Aging- San Francisco, CaliforniaApplyCompanioa is a new dementia offering from the Institute on Aging that aims to provide non-clinical services to individuals with dementia and their care partners. This offering includes providing an initial dementia-specific consultation with the Care Coach (either in home or virtually) that allows for a personalized recommendation plan. Following this consultation, IOA’s Care Coaches will be available to our clients virtually for education and support. This Care Coach role will develop relationships with a panel of clients and will be primarily reached by phone, chat, or email. Additionally, Companioa offers regular caregiver support groups, educational classes, as well as dementia-specific home care and day center services. The Care Coach will actively lead and participate in these education and support programs, as well as refer clients to other appropriate resources. Companioa is seeking a Care Coach based out of the San Francisco office to serve as CompanIioa’s primary contact with persons with dementia and their family caregivers, providing assessment, care coordination, support, and education services in the home and virtually.

Position Title: Life Stories Specialist

AWE is looking for volunteer biography writers, called "Life Stories Specialists," interested in supporting creative aging as a part of the Art With Elders Exhibits Program. Each year, Art With Elders selects 100 of our elder art students to be featured in our Annual Show. Each of the featured artists have their photo and a brief biographical sketch created and exhibited with their work. Our team of Life Stories Specialists interviews between 30 and 50 new program participants per year and creates 200-word biographical sketches about each artist’s life history and present experience with art class (link to http://www.artwithelders.org/annual-exhibit/ then click on “Artist Pages” ). The artist interviews are done in eldercare communities located in San Francisco, San Jose, and the East Bay, and are coordinated by AWE staff. We will begin student interviews in June and have finished biographies by end of September to be ready for our October Annual Show Event. Each elder artist is unique, some are living independently, others rely completely on caregivers because of physical and cognitive issues. Spanish and Mandarin translators are also needed. This year we are preparing for our 27th Annual AWE Show.
